REGISTER NOW for our "How to Get a Grant" course
Name:		Please fill out this form and click on send and mail a cheque for $95 to: The Sustainability Network, Att: Greenability Administrator, 615 Yonge Street, Suite 501 Toronto, ON, M4Y 1Z5. The administrator will confirm receipt of the registration form by email and save you a spot in the course. Submitting the online form will reserve you a spot in the course for approximately one week. When we receive your payment, we will contact you by email to confirm your enrollment in the course. You will be provided your user ID, password and a welcome message approximately 48 hours before the start of the course.

* Please fill out one form per person.
* Make the cheque payable to the Sustainability Network.
* Please ensure the cheque has your name on it as well as any sponsoring organization.
* If an organization is registering multiple students, please ensure this is clear in your communications.
* Visa or Mastercard payments can be faxed to 416 352-5470. Include your name, address, phone number, email, the card number, the expirty date, the name as it appears on the card and the number of participants. Your card will be debited for $95 (Canadian) per participant.
* Receipts will be provided on request.
